If you have a child who has passed, you may apply for a virtual online Loving Memories quilt by Smile Quilts for that child.

We do take requests for stillborn children of families who already have a child who has a Smile Quilt. We are very sorry, but at this time, we are not taking requests for ectopic or miscarried children outside of the Smile Quilts family.

If you are applying for a quilt for an angel child that is not your own child, we will have to have permission from the parents or guardians in order to make a Loving Memories quilt.

Please include a website address for the child, if there is one.

If you would like photos of friends or family members included on the quilt, please send them by email to Loving Memories by Smile Quilts, or if on a website, please grant Smile Quilts permission to use those photos.

Holiday, and monthly birthday pages are made for our Smile Quilts family at the appropriate time. If you do not celebrate traditional holidays or birthdays, please notify us of that and include in the message on the Request Form.

All information you supply is held as confidential and used only in the process of making a quilt to fit the specific needs of each family.

Please ... keep your email address updated with us if any changes are made. This is the only way we have to contact you with updates to the quilts or special event squares.
