If you have a child or know of a child who has a lifelong illness or condition, or who is terminally ill, you may apply for an online Virtual Smile Quilt for that child.
If you are applying for a quilt for a child that is not yours, we will have to have permission from the parents or guardians in order to make the child a quilt.
Also include web page address if your child has one.
If you would like your child's photo or friends and family member's photos included in the quilt, please send photos by email to Smile Quilts or if on a Web Site, please grant permission to use those photos.
Holiday pages and pages celebrating the child's birthday are presented at the appropriate time. If you do not celebrate traditional holidays or birthdays please notify us of that and include in the message of the form.
All information is confidential and used only in the process of making a quilt to fit the specific needs of each child.
